In accordance with the Federal Government’s Affordable Care Act regulations, HealthAlliance of the Hudson Valley recently completed the development of a Community Health Needs Assessment (CHNA) and corresponding Implementation Plan for Ulster and Delaware Counties.
HealthAlliance, the counties’ Department of Health, educational institutions and not-for-profit community organizations, such as Live Well Kingston, held public forums, community meetings and private sessions to gather data and complete the assessment. This collaborative effort led to the identification and prioritization of health needs that are afflicting the community at large.
Now, these entities are working to address the identified health needs in the community within at least two pre-defined NYS prevention agenda priorities: Chronic Disease Prevention and Promote Mental Health & Prevent Substance Abuse. The hospital’s Community Service Plan and Implementation Plans outline what activities HealthAllliance of the Hudson Valley will employ to enhance public health in these areas.
